About 21% of Walker County's residents were 65 or older in 2025, the highest share in the seven-county area. ADPH lists four assisted living and two specialty care licenses, all in Jasper, plus five nursing homes, five home health agencies and seven hospices. M4A handles aging services.
An older county with a compact care market
Walker County had 65,140 residents on July 1, 2025 in the Census Bureau's estimates, and 13,726 of them, about 21.1%, were 65 or older. That is the highest share among the seven counties we cover, ahead of Blount (19.9%) and St. Clair (19.5%), and well above Jefferson (18.7%). About 1,350 Walker County residents were 85 or older, the age group most likely to need daily help.
Licensed care is concentrated in Jasper, the county seat, which also holds Baptist Health Walker Hospital, the county's general hospital on ADPH's roster. Families in Cordova, Carbon Hill, Sumiton and Oakman generally look to Jasper first and to Jefferson County second.

The memory care supply is small: two group-class SCALF licenses with 30 beds between them. A family that needs secured memory care and cannot get a Walker County opening will usually look at Gardendale, Birmingham or Cullman. Our Walker County page lists every license with its newest posted ADPH inspection.
Small group homes are a big part of the picture
Three of Walker County's four assisted living licenses are group homes licensed for 16 residents, the top of Alabama's group class. Group homes follow the same admission, medication and resident-rights rules as large communities, but daily life is closer to a household: a few staff members, shared living rooms and a set routine.
Two of those group homes in Jasper share an administrator of record and were surveyed by ADPH on the same day in 2022, which is common for sister homes. When you compare them, read each license's own statement of deficiencies, because each building is licensed and inspected separately.
Nursing homes and CMS flags
Of Walker County's five nursing homes, CMS's October 2026 Care Compare file lists one, Walker Rehabilitation Center in Carbon Hill, as a candidate for its Special Focus Facility program, the program reserved for homes with a persistent record of serious quality problems. A candidate status is a reason to read the home's inspection reports closely and ask direct questions, not an automatic disqualification.
Each nursing home page on our site shows CMS's overall, inspection, staffing and quality ratings, the latest deficiency count and the five closest alternatives by distance.

For suspected abuse, neglect or exploitation, DHR's Adult Abuse Hotline is 1-800-458-7214, and the Walker County DHR office investigates. Complaints about an assisted living facility go to ADPH at 1-866-873-0366.
Hospital and rehab patterns
Baptist Health Walker Hospital in Jasper is the county's general hospital on ADPH's roster, and many residents are also treated in Birmingham. After a hospital stay, ask whether a rehab bed is available at one of the county's nursing homes so family can visit easily, and compare those homes' CMS ratings and staffing on our nursing home pages before choosing.
For families who live in Birmingham and have a parent in Walker County, the drive along Interstate 22 makes Jasper reachable, but weigh the weekly round trip honestly before choosing a community far from the people who will visit most.
Ask each Jasper community, too, how it coordinates with the hospital's case managers, because discharges often happen on short notice.
Planning ahead in a small market
With 30 licensed memory care beds in the county, openings can be scarce. Families caring for a parent with dementia in Walker County do well to tour both Jasper SCALFs early, ask about waitlists, and identify a backup in Gardendale or Birmingham before a crisis forces the choice.
