TL;DR

Hoover, Helena, Trussville and Leeds each extend into two or three counties. The county, not the city, decides the area agency on aging and the DHR office for Adult Protective Services. Licensing, inspections and Medicaid rules are statewide and do not change at the line.

Four cities, nine county pieces

Hoover straddles Jefferson and Shelby counties. Helena is mostly in Shelby County with a portion in Jefferson. Trussville is mostly in Jefferson County and extends into St. Clair. Leeds touches three counties: Jefferson, St. Clair and Shelby. For a family settling a parent in one of these cities, the street address decides the county, and the county decides several of the agencies involved.

Why the area agency on aging matters

The area agency on aging runs the Aging and Disability Resource Center, SHIP Medicare counseling and the long-term care ombudsman program for its counties, and in M4A's region it also handles Elderly and Disabled and ACT waiver case management. The United Way Area Agency on Aging of Jefferson County (205-458-3330) covers Older Americans Act services in Jefferson County; M4A (205-670-5770) covers Shelby, St. Clair, Blount, Walker and Chilton counties.

If you are unsure which side of the line an address is on, call 1-800-AGE-LINE; it routes by location. When a parent moves from a Jefferson County apartment to a Shelby County assisted living community a few miles away, update the ombudsman contact you keep on file.

Adult Protective Services goes by county too

Alabama's Department of Human Resources runs Adult Protective Services through county offices. The statewide Adult Abuse Hotline, 1-800-458-7214, takes the report and routes it, but the investigating office will be the one for the county where the adult lives. Give the full street address when you call.

What does not change at the county line

The rules a facility must follow are statewide. Every assisted living facility in Alabama is licensed by the Alabama Department of Public Health under chapter 420-5-4, every memory care unit with secured exits needs a specialty care license under chapter 420-5-20, and nursing homes are licensed under chapter 420-5-10. ADPH's complaint lines are the same everywhere: 1-866-873-0366 for assisted living and 1-800-356-9596 for nursing homes.

Medicaid eligibility, the 2026 limits and the waiver programs are statewide as well. A county line does not change what a parent qualifies for.

Practical tips for border cities

Search ADPH's directory by both counties when you look for facilities near a border city; filtering by one county will miss licenses on the other side. Our city pages for Hoover, Helena, Trussville and Leeds rank licensed options by distance from the city center rather than by county, so they show both sides at once.

Hospital choices also ignore county lines. A parent in Leeds may be closer to a Jefferson County hospital, while one in southern Helena may be closer to Baptist Health Shelby in Alabaster. Check distances before you choose a community you will visit weekly.

Hospitals and emergency response

Emergency services generally follow the jurisdiction where the call originates, and the closest hospital may sit in a different county from the parent's home. For families comparing communities in border cities, the straight-line distance to the nearest general hospital on ADPH's roster is a useful first check; our city pages list the three nearest for each city.

Ask each community which hospital its residents are usually taken to in an emergency, and whether a resident or family can request a different one.

When paperwork asks for a county, use the county of the physical address, not the mailing city. That one detail prevents most of the misrouted calls families describe to us, especially in Hoover and Leeds.

If a parent moves into a facility across the line, update the address on file with Social Security, Medicare and any pension so notices reach the right place.

Voting, taxes and other county services

A move across a county line also changes the county probate office, which handles guardianship and conservatorship petitions, and the county revenue office for property tax questions such as homestead exemptions for older homeowners. If a parent keeps a home in one county while living in assisted living in another, note both offices in your family records.

Frequently asked questions

Which area agency on aging serves Hoover?

It depends on the address: the Jefferson County side is served by the United Way Area Agency on Aging and the Shelby County side by M4A. 1-800-AGE-LINE routes callers by location.

Is Leeds in Jefferson County?

Partly. Leeds spans Jefferson, St. Clair and Shelby counties.

Do assisted living rules differ between Jefferson and Shelby counties?

No. ADPH licenses and inspects assisted living under the same statewide rules in every county.

Where do I report elder abuse in a border city?

Call DHR's Adult Abuse Hotline at 1-800-458-7214 with the full address; the county office where the adult lives investigates. Call 911 in an emergency.
