TL;DR

ADPH lists 17 assisted living and memory care licenses in Shelby County, and several in the U.S. 280 and Riverchase areas use Birmingham mailing addresses. For aging services those addresses belong to M4A in Alabaster, not the Jefferson County agency, and searching ADPH by county rather than city finds them all.

A Birmingham address that is not in Jefferson County

Drive out U.S. 280 past Interstate 459 and you cross into Shelby County long before the mailing addresses stop saying Birmingham. ZIP code 35242, which covers Greystone, Inverness and much of the corridor, is a Birmingham postal area that sits largely in Shelby County. The same thing happens around Riverchase, where Hoover straddles the county line.

For senior care that detail matters in three ways. It changes which area agency on aging helps the family, which county Department of Human Resources office handles an Adult Protective Services report, and how you search the Alabama Department of Public Health's directory, which sorts by county.

What ADPH's roster shows in Shelby County

ADPH's directory export of October 30, 2026 lists 17 assisted living and specialty care (memory care) licenses in Shelby County. Ten are regular assisted living licenses and seven are specialty care. Four of the ten regular licenses are 16-bed group homes in Chelsea, Pelham and Helena, one is classed by ADPH as a family facility, and the other five are congregate communities with 38 to 87 beds along the 280 corridor and in Riverchase.

The table shows the larger corridor licenses; our Shelby County page lists all 17 with their newest posted inspection dates. Several campuses hold two licenses at one address, an assisted living license and a separate memory care license, each inspected on its own.

Which agency helps a family in the 280 corridor?

Older Americans Act services in Shelby County, including the Aging and Disability Resource Center, SHIP Medicare counseling, the long-term care ombudsman and Medicaid waiver case management for the Elderly and Disabled and ACT waivers, come from the Middle Alabama Area Agency on Aging (M4A), based on Cloverdale Circle in Alabaster. Its main number is 205-670-5770 and its toll-free line is 1-866-570-2998.

A parent living in a Greystone community with a Birmingham address is therefore an M4A client, not a client of the United Way Area Agency on Aging of Jefferson County. If you are unsure, 1-800-AGE-LINE routes the call by location.

Searching the state roster the right way

ADPH's Health Care Facilities Directory lets you filter by county or by city. Filtering by the city of Birmingham alone returns facilities in both Jefferson and Shelby counties, while filtering by Shelby County returns the corridor communities whatever their mailing city. For a complete picture of the 280 corridor, search Shelby County and Jefferson County separately and combine the results, which is what our roster does.

Hospital proximity follows the map, not the address. On ADPH's hospital roster, Baptist Health Shelby Hospital in Alabaster and the Brookwood and Grandview hospitals in Jefferson County are the general hospitals families along the corridor most often use, depending on where along 280 they live.

Questions to ask corridor communities

Ask which license covers the apartment or room you are considering, assisted living or specialty care, and read that license's newest ADPH statement of deficiencies. Ask how a move from the assisted living building to the memory care neighborhood is handled and priced. And ask about traffic: a community ten miles away on 280 at rush hour may be farther in practice than one fifteen miles away on Interstate 65.

Shelby County had about 43,300 residents 65 and older on July 1, 2025, according to Census Bureau estimates, the second-largest older population in the seven counties after Jefferson. Demand along the corridor is real, so ask about waitlists early.

Campuses with two licenses

Several corridor communities hold an assisted living license and a specialty care license at the same address. On a tour, ask which building or wing each license covers, and read both inspection reports, since each license is surveyed separately.
